A main surgery for the practice and a focal point for health care advice, treatment, information and community support

Description: 2 GP and nurse practitioner primary care premises including treatment room and multi-purpose room

Project brief: The Wye Valley Practice runs General Medical Services (GMS) from 2 sites: one in St Briavels and one in Trellech. The GMS in Trellech is currently provided from privately owned leased premises with very cramped conditions and as a consequence of its size, any increase in the range of primary and community based services within the existing premises is limited and offers no scope for service expansion. It is anticipated that the new development in Trellech will become the main surgery for the practice and a focal point for health care advice, treatment, information and community support. The focus will be to provide a modern, clean, flexible environment which will lead to improved access to services, an extended and integrated range of services, sustainability of service delivery,
improved efficiency and reduce health inequalities.

Project solution: The proposal for the scheme has been developed with discussions involving the Local Health Board, Welsh Health Estates, District Valuer, Monmouthshire County Council, Community Health Council, Social Services, Gwent Healthcare Trust, Ashley House plc, the GPs and practice staff and patients. Agreement has been reached for a single storey new surgery at a council owned site within the village to be constructed by Ashley House to modern standards, taking into account sustainability and good design principles. In deciding to implement this proposal, the LHB believe that this will provide the optimal solution not only for health services locally, but for the long term sustainability of the primary care estate.

Points of interest: The current surgery has a gross internal area of 120m2. The original building dates back to the 1920s and was owned by the Waterworks before being converted to a surgery in 1977. The surgery was extended and improved in 1996, but facilities are still extremely restricted and the property is not connected to a main drainage system. Trellech is in a rural location with limited public transport. The patient catchment area covers a wide area taking in two of the major towns of Monmouth and Chepstow. The population of Monmouth has increased by 20% in the last two decades and Trellech and the surrounding area are much sought after as a commuting base to London, Bristol and Cardiff which has a significant affect on the planned healthcare provision of the Trellech area. There is a high rate of mortality caused by stroke in particular in the Trellech (and Shirenewton) areas, a higher prevalence of mortality from respiratory disease compared to other localities and an above average number of people registered as being hypersensitive, further demonstrating the need for improved premises to accommodate new services to address these issues.
